I have a coworker with a medical condition that causes him to have an odd pulse, and which looks misleading on a chest X-ray. They did one of those procedures where they insert a tiny camera in the femoral artery and go take pictures, which is how they found out it wasn't what it looked like on the X-ray.

His doctor has suggested that he get a tattoo on his chest directly over his heart with a brief message giving the name of the condition. So, should he end up in an ER unable to speak and they do an emergency X-ray, they don't cut him open to fix something which isn't what they think it is.

How does that potential direct harm weigh against the prohibition in question?

In Jewish philosophy, life is paramount. If getting a tattoo could help address a serious medical condition, it is by all means permissible.
